How to get the intake form
Email us at frontdesk@deafcounseling.com and we will reply with a secure link to the client intake form. You can complete it on a phone, tablet or computer, and you do not need to print or scan anything.
If you would rather not use email, text us on 240 701 5155 or reach us by videophone on 240 380 2577 and we will send the link whichever way suits you.
The form asks for the basics we need before a first session — contact details, what brings you to therapy, and how you prefer to communicate. It takes about ten minutes.
Before your first session
Completing the intake form ahead of time means the first session can be about you rather than paperwork. If anything on it is unclear, leave it blank and we will go through it together.
